Patient was noted to have right sided wing droop at morning check. Pulled in afternoon for radiographs.

As radiographs revealed NEW fracture of ulna not at repair sites, patient will be kept in body wrap for next 2-3 weeks and recheck radiographs taken. There is not significant enough displacement of the fracture fragments to warrant surgical intervention at this time.

Placed back on medications that patient was previously on and added buprenorphine for new acute fracture. Placed in body wrap.

Took VD and caudocranial radiographs of right radius/ulna as it was reported that 276 was holding wing abnormally in aviary. On capture, it was noted that there was some crepitus mid-ulna. On radiographs there is a new oblique moderately displaced fracture of the middle fragment of the previous segmental fracture (new fracture, not at the two previous fracture sites of the segmental fracture).

Patient had fixator bar removed today and PT of wing performed during removal.
Induced under iso 5%, reduced to 2% and maintained on mask. Patient had wing placed through full range of motion about ten times. Fixator bar broken with metal clippers, then pins removed with pliers. Pin sites closed with tissue glue. Wing palpated as very stable. Patient held until recovered fully.

Patient was taken out to LFA1 and released, flew when released and seemed to fly reasonably well. Oral meds were d/c as patient moved outside.

VD and skyline/craniocaudal projections obtained. There is bridging bone callus formation across both fracture sites of segmental ulnar fracture. There is proliferation of bone at all pin sites. There is displacement of the middle portion of the segmental fracture that is worse compared to initial radiographs and this has led to proliferation of bone on distal portion of middle fragment that is very close to the radius but not yet causing synostosis. This is visible ventrally as a large bony nodule near the radius on physical exam. Right now, supination/pronation appear intact. Based upon radiographs, fully healed and fixator can be removed, need to monitor for synostosis between radius and ulna, removal of pins may aid in reduction of further bone hypertrophy and reduce chances of synostosis, remodeling of bone expected with flight may do the same. Recommend recheck rads of wing if flight does not return to expected ability to determine if synostosis present

Incisions from surgical repair of right ulna fracture are healing well. No separation of incision edges or excessive redness, swelling or discharge. Acrylic tubing cracked adjacent to the middle ESF pin. Reapplied acrylic to reinforce/stabilize tubing. Changed bandage and applied carpal bumper and wing-to-body wrap of right wing. Changed carpal bumper on left wing.

Procedure - Fracture repair - ulna

Surgery time - 120 minutes

1. Whole body radiographs
2. Brachial plexus block with 6 mg bupivacaine
3. Identified leak in ventilation system, extubated and switched
to 7.0 mm ET tube
4. Ulnar fracture repair

- Fracture Repair - 
The patient was placed in sternal recumbency. The ulna had been
fractured in 2 places (mid-diaphysis and distal diaphysis). A
fibrous callus had formed at each fracture site. A 1.5 cm skin
incision was made over the distal ulna using a #15 blade scalpel.
Tonometry scissors were used to bluntly dissect down to the ulna.
A closed reduction was attempted, and adequate apposition of
the distal fracture fragments was confirmed via fluoroscopy.
The same steps were performed for the mid-diaphyseal fracture.
A 1.5 cm skin incision was made over the mid-diaphysis of the
ulna using a #15 blade scalpel. Tonometry scissors were used
to bluntly dissect down to the ulna. A closed reduction was attempted,
and adequate apposition of the mid-diaphyseal fracture fragments
was confirmed via fluoroscopy. Five 2.0 mm transfixation pins
were placed through both cortices unilateral/uniplanar along
the length of the ulna. Positioning of the pins was confirmed
via fluoroscopy. The skin incisions were closed with 4.0 PDS
in a simple continuous pattern.
